this form is completed, a response will be securely emailed or faxed within 24 business hours.

California Reportable Disease Information Exchange (CalREDIE) Provider Portal

The CalREDIE Provider Portal provides secure access for healthcare providers to electronically submit Confidential Morbidity Reports (CMRs), required by California Code of Regulations (CCR), Title 17, Section 2500 directly to Local Health Departments (LHD). To register for CalREDIE Provider Portal access, return a completed CalREDIE Provider Portal User Account Authorization Form to SexualHealth@acgov.org.

Confidential Morbidity Report (CMR)

In the State of California, health care providers who have diagnosed, or suspect the presence of, a sexually transmitted disease (STD) in their patient are legally required to report that information to the local health department. This form is designed for health care providers to report those diseases mandated by Title 17, California Code Regulation (CCR).

Sexually Transmitted Infections (STI) Treatment Guidelines (2021 Update)

The Centers for Disease Control and Prevention (CDC) has updated their STI Treatment Guidelines to provide current evidence-based prevention, diagnostic, and treatment recommendations that replace the 2015 guidance. The recommendations are intended to be a source for clinical guidance; healthcare providers should always assess patients based on their clinical circumstances and local burden.
