Program Eligibility

Care Partners is designed to be low barrier and assist Alameda County residents with supportive social services if they meet any one of the following criteria:

• In-Home Supportive Services (IHSS) Recipient or Care Provider
• Low-income older adults 50+
• Low-income people with disabilities
• Low-income, monolingual, or undocumented older adults
• Low-income LGBTQ+ older adults
• Care providers of those above/those in their Circle of Care
