Recall of Frozen Strawberries from Egypt Potentially Contaminated with Hepatitis A

A large recall of frozen Egyptian strawberries that have been distributed widely in the United States, including California, has been issued due to the possibility of hepatitis A virus (HAV) contamination. The U.S. Centers for Disease Control and Prevention (CDC), the U.S. Food and Drug Administration (FDA), and state and local officials have been investigating an outbreak of HAV linked to frozen strawberries imported from Egypt.

General Mills Has Expanded Its Recall of Flour Products Due to Possible E.coli Contamination

General Mills, a Minneapolis, MN establishment has expanded its recall of Gold Medal flour, Wondra flour, and Signature Kitchen flour due to possible E.coli O121 contamination.

Ghiringhelli Specialty Foods, Inc. is recalling "Trader Giotto’s Caesar Salad with Chicken Breast" due to misbranding and undeclared allergens

Ghiringhelli Specialty Foods, Inc., a Vallejo, California establishment is recalling "Trader Giotto’s Caesar Salad with Chicken Breast" due to misbranding and undeclared allergens – fish, soy, and wheat.  The salads have the correct label on the top of the package, but exhibit an incorrect back label for "Trader Joe’s Chicken & Roast Beet Salad."

CDPH Warns Consumers Not to Eat Bivalve Shellfish Caught in Humboldt or Del Norte Counties

California Department of Public Health (CDPH) Director and State Health Officer Dr. Karen Smith today advised consumers not to eat recreationally harvested bivalve shellfish (mussels and clams) from Humboldt or Del Norte counties. Only the white meat (adductor muscle) of scallops should be consumed and the viscera (internal organs) should be discarded.

Osamu Corporation Recalls Tuna Products Due to Possible Salmonella Contamination

Osamu Corporation, a Gardena, CA establishment is recalling frozen tuna products due to possible Salmonella contamination.

These tuna products were sold in bulk packaging to distributors and foodservice vendors for use in sushi sold at restaurants and in pre-packaged sushi rolls purchased at grocery stores

Whole Foods Market Recalls Made-In-Store Items Prepared With Stone Fruit Voluntarily Recalled by Wawona Packing co. Due to Possible Health Risk.

Whole Foods Market has recalled made-in-store items prepared with organic and conventional stone fruit, including peaches, nectarines, and plums from Wawona Packing Co. because of possible contamination with Listeria monocytogenes. While no illnesses have been reported to-date, Listeria monocytogenes is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems.

Foster Farms Recalls Chicken Due to Possible Salmonella Heidelberg Contamination

Leiter's Compounding Pharmacy: Recall - Concerns of Sterility Assurance

Leiter's Compounding Pharmacy is voluntarily recalling 3 lots of its sterile products due to concerns of sterility assurance with Front Range Laboratories, Leiter's Compounding Pharmacy's independent testing laboratory. The following products are subject to the nationwide recall:  Bevacizumab  & Lidocaine/phenylephrine.  The use of a non-sterile injectable product exposes patients to the risk of contracting serious life-threatening infections. Leiter's Compounding Pharmacy has not received any reports of adverse events or reports of contamination but decided to recall the products following a recent inspection by the FDA of Front Range Laboratories. These products were dispensed to health care providers between 8/05/13 to 9/02/13 nationwide throughout the United States.

FDA announces nationwide voluntary recall of all products for sterile use from Specialty Compounding

The U.S. Food and Drug Administration is alerting health care providers and patients of a voluntary nationwide recall of all products produced and distributed for sterile use by Specialty Compounding, LLC, Cedar Park, TX.  There have been recent reports of bacterial bloodstream infections potentially related to the company's calcium gluconate infusions.

All sterile use products produced and distributed by Specialty Compounding are being recalled and none of these products should be used by patients or administered to patients. Facilities, health care providers and patients who have received the products since May 9, 2013 should immediately discontinue use, quarantine the products, and return the products to Specialty Compounding.

According to information provided by the firm, the recalled products were distributed directly to patients nationwide, with the exception of North Carolina.  Recalled products were also distributed directly to hospitals and physicians’ offices in Texas.

Merck Hepatitis B Adult Vaccine Recall

Merck has initiated a voluntary product recall to their private customers who purchased Adult Hepatitis B Vaccine (RECOMBIVAX HB(r) Lot # J001183 of NDC 00006-4995-41. The recall is being initiated due to the potential for a limited number of cracked vials to be present in Lot J001183 of NDC 00006-4995-41. This lot was distributed by Merck between March 12, 2013 and May 2, 2013. The State has confirmed that these doses were not purchased or distributed through CDC mechanisms. If you receive questions about this recall from your partners and/or providers, these should be directed to the Merck National Service Center: 800-672-6372 (Select Prompt #1 then Prompt #2). Service Center hours are Monday to Friday 8 AM to 7 PM EST.

Voluntary Recall of All Ameridose Medical Products

On October 31, 2012, the Food and Drug Administration (FDA) announced that Ameridose is voluntarily recalling all of its unexpired medical products in circulation. Ameridose is based in Westborough, Mass., and is managed by some of the same people as the New England Compounding Center (NECC), the firm that distributed and recalled injectable medications implicated in the ongoing multistate outbreak of fungal meningitis and other infections.

Fungal Meningitis Outbreak: FDA Provides NECC Customer List

FDA is making available two lists of customers (consignees) who received products that were shipped on or after May 21, 2012 from New England Compounding Center’s (NECC) Framingham, MA facility. The first list includes customer names and addresses, organized by state; the second list is organized alphabetically by customer name and also includes the specific products shipped, the quantities of product shipped, and the shipping date.

Trader Joe’s Voluntarily Recalls Creamy Salted Valencia Peanut Butter Because of Possible Health Risk

Trader Joe’s is voluntarily recalling its Creamy Salted Valencia Peanut Butter (sku 97111) because of potential contamination with Salmonella. Customers who have purchased this item are urged to not eat the product, and to dispose of it or return it to any Trader Joe’s for a full refund.

CDPH Warns Not to Eat Red Vines® Black Licorice, Snaps®, and Mixed Bags containing Black Licorice Candy

California Department of Public Health (CDPH) warned consumers not to eat Red Vines® Black Licorice  Twists, Family Mix, Mixed Bites and Snaps containing black licorice. This warning comes after the manufacturer expanded its August 22 recall because it determined these products may contain levels of lead exceeding the state’s standards. Consumers in possession of the candy should discard it immediately.

CDPH Warns Not to Eat Certain Red Vines® Black Licorice Candy

Dr. Ron Chapman, director of the California Department of Public Health (CDPH) and state health officer, warned consumers not to eat Red Vines® Black Licorice Twists candy, ‘Best Before’ 020413, after tests conducted by CDPH found the products contained levels of lead that exceeded the state’s standards. Consumers in possession of the candy should discard it immediately.

Tanimura & Antle Recalls Romaine Lettuce Due to Possible E Coli Contamination

Tanimura & Antle Inc. is voluntarily recalling a single lot of romaine lettuce because it may be contaminated with Escherichia coli O157:H7 bacteria (E. Coli O157:H7). The affected product is limited to Tanimura & Antle Field Fresh Wrapped Single Head Romaine packed in a plastic bag with the UPC number 0-27918-20314-9 and may have a Best Buy date of “08 19 12”.

CDPH Warns Not to Eat Oysters From Drakes Bay Oyster Co.

California Department of Public Health (CDPH) director Dr. Ron Chapman today warned consumers not to eat shucked and in-shell raw oysters from Drakes Bay Oyster Company because they may be contaminated with Vibrio parahaemolyticus, a bacterium that can cause serious illness. To date, three illnesses in California have been linked to the products.

Banner Mountain Sprouts Voluntarily Recalls Organic Sprouts Because of Possible Health Risks

As a precautionary measure, Banner Mountain Sprouts is voluntarily recalling organic sprouts under the following names:
4oz. zesty greens, 5oz. sprout salad, 4oz. clover, 2lb. clover, 4oz. alfalfa/broccoli, 4oz. alfalfa sprouts, and 1lb. & 2lb. alfalfa sprouts because they may be contaminated with salmonella. Routine testing showed signs of salmonella and Banner Mountain Sprouts was notified of possible contamination.

Update Regarding the Sourcing of Molluscan Shellfish from Korea

Oysters, clams, mussels, and scallops from Korea should be removed from the market.

On May 1, 2012, the Food and Drug Administration (FDA) removed all Korean certified shippers of molluscan shellfish (oysters, clams, mussels, and scallops) from the Interstate Certified Shellfish Shippers List (ICSSL) following a comprehensive FDA evaluation that determined that the Korean Shellfish Sanitation Program (KSSP) no longer meets the sanitation controls spelled out under the National Shellfish Sanitation Program.

Santos Agency, Inc. has initiated a recall of Santos Brand products i.e. various kinds of flours, legumes and spices because they may contain undeclared peanuts and wheat.  Consumers who are allergic to these items may run the risk of serious or life-threatening allergic reactions if they consume any of the products.

Pennsylvania Firm Recalls Lamb Koftis Due to Misbranding and an Undeclared Allergen

August Foods Inc., a Downington, Penn. establishment, is recalling approximately 20,520 pounds of lamb koftis, the U.S. Department of
Agriculture's Food Safety and Inspection Service (FSIS) announced today. The lamb koftis contain milk, a known allergen that is not declared on the product label.

Funfresh Foods, Inc. Voluntarily Recalls World Berries™ Organic “Cacao Nibs” Because of Possible Health Risk

FunFresh Foods of San Clemente, CA, in cooperation with the FDA, is voluntarily recalling a single lot of its 6 oz. packages of FunFresh Foods World Berries Organic “Cacao Nibs” because they may be contaminated with Escherichia coli 0157:H7 bacteria.  E. coli 0157:H7 causes a diarrheal illness often with bloody stools.

CA Dept. of Food and Agriculture Announces Recall of Raw Milk Products at Claravale Farm, San Benito County

Raw milk, raw nonfat milk and raw cream produced by Claravale Farm of San Benito County is the subject of a statewide recall and quarantine order. The quarantine order came following the confirmed detection of campylobacter bacteria in raw cream.

Walgreen Co. Voluntarily Recalls Certain Lots of 13 Oz. Chocolate-Covered Raisins As Product May Contain Peanuts, Almonds, Soy Due to Packaging Error

Walgreen Co. is voluntarily recalling certain lots of 13-oz. Walgreen’s Chocolate-Covered Raisins because the packages may contain Walgreen’s Bridge Mix with peanut, almond and soy ingredients. The error occurred when Walgreen’s Bridge Mix was mistakenly packaged with the Walgreen’s Chocolate-Covered Raisin labeling.

December 30, 2011 Voluntary Recall of Limited Quantity of 6-pack 250 mL Martinelli’s Gold Medal Sparkling Cider

As a precautionary measure, certain production lots of Martinelli’s Gold Medal® Sparkling Cider in six-pack shrink-bundled 250 mL glass bottles are being recalled in the Western United States due to the possibility of a defective seal that could break when opening the bottle.

The six-pack shrink-bundled 250 mL Martinelli’s Gold Medal Sparkling Cider “Best By” dates being recalled are:

These production codes can be found on the front, lower corner of the bottle label under the words “BEST BY:” in a gold colored box. They are for case UPC 4124499932, six- pack UPC 4124446256, and single bottle UPC 4124400256.

Consumers who have purchased the recalled products are asked to return the product to the store where it was purchased for product replacement or credit.

Note: The 750 mL size of Martinelli’s Gold Medal Sparkling Cider is NOT affected and is NOT part of this recall.

New Zealand exporter Talleys Group Ltd has undertaken a voluntary recall on Greenshell Mussels packed at Talleys Group Ltd, Blenheim (premises number PH159). Product packed on and after 11/2/11 may be affected by abnormally high generic Escherichia coli.
November 17, 2011 CDPH Warns Consumers Not to Eat Certain Ready Pac Brand Romaine Lettuce
California Department of Public Health (CDPH) is warning people not to consume bagged salad products from Ready Pac Food Inc. because of romaine lettuce possibly contaminated with E coli O157:H7. No illnesses have been associated with the recalled product.
November 15, 2011 Organic Pastures Raw Milk Recall Announced by CDFA
All Organic Pastures raw dairy products with the exception of cheese aged a minimum of 60 days are to be pulled immediately from retail shelves and consumers are strongly urged to dispose of any products remaining in their refrigerators.
February 7, 2011 Recall Expanded for 'Nuclear Sludge' Candies
Director of the California Department of Public Health Dr. Mark Horton is warning consumers to avoid all flavors of “Toxic Waste” brand “Nuclear Sludge” candies. The candies were found to contain high levels of lead, which is especially toxic to infants, young children and pregnant women.
September 21, 2010 Ginger Candy Recall
Today, the California Department of Public Health (CDPH) warned consumers not to eat Ginger Candy imported from China after tests conducted by CDPH found unacceptable levels of lead. Consumers in possession of the candy should discard it immediately. The candy with the brand name “Ginger Candy” contained as much as 0.25 parts per million of lead. Pregnant women and parents of children who may have consumed this candy should consult their physician or health care provider to determine if medical testing is needed.
August 13, 2010 Egg Recall
Wright County Egg of Galt, Iowa is expanding its voluntary recall (initiated August 13, 2010) of specific dates of shell eggs produced by their farms because those eggs have the potential to be contaminated with Salmonella.

The Food and Drug Administration (FDA) continues to post voluntary recalls of products potentially contaminated with Salmonella typhimurium from peanut butter and peanut paste recalled by the Peanut Corporation of America.
