Eligibility

A person is eligible to participate in the Asthma Start Program if (s)he meets ALL of the following requirements:

• is between the ages of 0-18
• has been diagnosed with asthma
• lives in Alameda County

Medical providers, school personnel, parents, and guardians can refer their patient or child that meets all the eligibility requirements. Please complete and return this referral form or contact the Asthma Start Program at (510) 383-5181.
