Zika Disease Reporting Numbers
- REPORT suspected or confirmed cases of dengue, chikungunya, or zika virus disease to Public Health Acute Communicable Disease Unit (ACD) by faxing a Confidential Morbidity Report form to 510-273-3744.
- FAX the ACPHD ZIKA VIRUS TESTING REQUEST FORM to PUBLIC HEALTH at 510-273-3744 to obtain approval prior to sending specimens.
- During Business Hours (8:30 am- 5:00 pm Monday-Friday)
