Children and youth may be able to get free CHDP health check-ups.  Find a CHDP provider office from the list below. A CHDP 1-page form will need to be filled out at the provider office. With the CHDP Gateway, uninsured children can see a doctor and receive temporary Medi-Cal at the time of their health exam. See Steps to Getting a Free CHDP Exam for more information.
