Public Health Nursing
The Alameda County Public Health Nursing, staffed by public health nurses and community health outreach workers, delivers an array of health and wellness services, health education and outreach to individuals, families, and communities throughout Alameda County. These services are delivered in homes, clinics, community centers, schools, and other locations.
Services include:
- general case management, health education and physical assessment
- immunizations
- information about preventive health care
- assistance with scheduling medical appointments
- assistance accessing diagnostic and treatment services
- medical referrals

The Alameda County Public Health Nursing Division provides public health nursing care through community outreach, home visiting, care coordination and advocacy to address individual and community health needs, promote healthy living, eliminate health disparities, improve health outcomes, and ensure optimal quality of life for all Alameda County residents.
